NAGPUR: Malaria is often classified depending on the kind of parasite that causes the disease. A scientist from Indian Council of Medical Research (ICMR), however, has categorised the disease on the basis of the lifestyle of the affected population.

Neeru Singh, director of Jabalpur regional centre of ICMR, believes that research as well as prevention of the disease could be more focused if factors causing malaria are taken into account instead of vectors. She has grouped the disease under the heads tribal and forest malaria, industrial malaria, migratory malaria and urban malaria.

"The living conditions and surroundings of the people are responsible for spread of the disease. If we were to implement measures to address those causes, we would be hitting at the foundation of spread of malaria," said the scientist who specializes in malariology and is a member of Indian Society for Malaria and other communicable diseases.

Having worked for the tribals in Chhattisgarh and Madhya Pradesh, she has done vast research on tribal and forest malaria. "The tribals account for around 9% population of the country but 30% of the total cases of malaria, most of which fatal. The huge incidence of the disease amongst them is largely due to their habitat - the forests, which become unapproachable for some months in a year," she informed.

Forest dwelling population in other nations of the world is also affected by the disease in large numbers. In fact, the World Health Organisation has launched an enhanced malaria control programme, with financial assistance from the World Bank , especially focusing on forest malaria.

Industrial malaria refers to the disease in the cluster of industrial workers. "Very often, industries are situated at places away from cities. So, the workers build temporary shelters in unhygienic conditions with their waste disposed of in an improper way very close to the colony, causing mosquitoes to breed near homes," explained Singh.

The story of migratory malaria is similar. Migrants do not find proper shelter easily, especially in cities. They end up living in slums that have unhealthy living atmosphere. The difference here is that these people mix with others living in the same city, posing a danger to their health as well. Those living in cleaner houses may also be prone to malaria due to faulty practices. Mosquitoes may breed in their surrounding because of stagnant pools of water in coolers, fountains or even vases.

The Bill and Melinda Gates Foundation lists malaria prevention as one of its main causes, funding an anti-malaria vaccine that is still in development. Besides a vaccine, simple protective gear such as insect nets would make a tremendous difference in the lives of those most vulnerable to malaria in Africa and Asia.

However malaria still thrives as a plague of the poor. While there is no vaccine, there are pills to help prevent malaria, but are usually only available to travelers from wealthier nations. Treatment for malaria is often too expensive for the poor, and their bodies can often develop a resistance to it. Meanwhile, about 2.7 million people die from the disease each year, the vast majority being children.

Last week Australian researchers announced they had discovered a new way to treat malaria by deactivating an enzyme that parasites use to feed on red blood cells. While this is great news, the question of accessiblity to the poor, and most affected, still remains. This is a question for those in power, namely governments and pharmeceutical companies, who will somehow need to put aside their desire for profit in the name of humanity and social justice.

Besides Gates, Bill Clinton and his Clinton Foundation have also joined the fight against malaria. Show your support in their fight to make treatment more accessible to the poor by adding your signature to the petition.

Medicine is important in the struggle against maleria, but prevention is even more important. Mosquito nets for all and elimination of breeding grounds would go much further in preventing maleria than any kind of medicine. These efforts would also have the side effect of helping stop the spread of denge, another deadly, mosquito-borne disease.
